What is gingivectomy?

Gingivectomy is a procedure during which a dentist carefully removes excess gum tissue. It is needed when the gums grow too actively, interfere with the teeth, or form “pockets” where bacteria can easily accumulate. Thanks to this, tooth care becomes easier, the risk of inflammation and bleeding is reduced, and the smile looks neater and more natural.
The procedure is usually performed under local anesthesia, making it practically painless, and recovery occurs fairly quickly. In addition, gingivectomy is often used before orthodontic treatment or prosthodontics to provide convenient access to the teeth and make care easier after the installation of braces or crowns.
It is also worth mentioning laser gingivectomy separately. This is a specific type of gingivectomy where excess gum tissue is removed using a laser. The laser seals the blood vessels during operation, making the procedure less painful, almost bloodless, and healing faster. The indications and contraindications are the same as for classical gingivectomy.
Indications and contraindications for the procedure
Gingivectomy can be beneficial in various situations where surgical intervention is required to improve oral health and smile aesthetics:
- Excessive gum growth
When the gums cover the teeth too much, it makes cleaning them difficult and promotes the accumulation of plaque and bacteria. In such cases, removing part of the gum makes tooth care simpler and more effective. - Gum inflammation. In cases of chronic gingivitis or early-stage periodontitis, other treatment methods, such as professional cleaning or medication therapy, may not yield the desired effect. Gingivectomy helps reduce inflammation and facilitate hygiene, preventing the disease from progressing.
- Aesthetic issues. Uneven or excessively large gums can make a smile non-harmonious. During the procedure, the doctor aligns the gum contour, which allows for achieving a more aesthetic and natural look for the smile.
- Preparation for orthodontic treatment. If the installation of braces or other orthodontic appliances is planned, a gingivectomy helps provide easier access to the teeth and ensures an even fit of the appliance.
- Preparation for prosthodontics. For the installation of crowns or bridges, it is important that the gums do not interfere and that the prostheses fit correctly. Removing excess tissue provides convenient access and contributes to the longevity of the prostheses.
The procedure can be dangerous or may need to be postponed in the following cases:
- Acute infections in the mouth or respiratory tract. Intervention during an active infection period can lead to complications during the procedure and worsen post-operative healing.
- Serious heart or blood conditions. In patients with severe cardiovascular diseases or blood clotting disorders, the risk of bleeding and post-operative complications increases.
- Immune system disorders or wound healing issues. In chronic illnesses, such as diabetes, or other conditions affecting immunity, the healing process may slow down.
- Pregnancy, especially the first trimester. Due to stress on the body and potential risks from anesthesia, surgery during this period is usually postponed.
- Severe gum bleeding. Before surgical intervention, the causes of bleeding must be addressed first to ensure the procedure is performed safely.
Gingivectomy is an effective procedure for improving gum health, tooth care, and smile aesthetics. At the same time, it is important to consider the indications and contraindications to ensure a safe intervention and a rapid recovery. Before the procedure, it is mandatory to consult with a dentist to select the optimal approach specifically for your case.
Gingivotomy and gingivectomy: what’s the difference?
Gingivotomy and gingivectomy are two different dental procedures that involve the gums, but they have distinct goals and methodologies.
Gingivotomy is a surgical intervention during which a dentist makes an incision in the gum. The goal of this procedure is to facilitate access to the tooth or a periodontal pocket, relieve pressure on the tissue, and promote the treatment of gum inflammation. In a gingivotomy, a large portion of the gum is not removed — it remains in place, and only its shape is altered or the space is widened to improve hygiene. This procedure is often used in periodontitis, when pockets that are difficult to clean form between the teeth and gums.
Gingivectomy, conversely, involves the removal of a portion of the gum. The primary goal is to eliminate excess tissue that covers the teeth or creates aesthetic problems. A gingivectomy not only helps improve the appearance of the smile but also makes tooth care more convenient, reduces the risk of inflammation and bleeding, and prepares the teeth for orthodontic treatment or prosthodontics.
Simply put:
- Gingivotomy is an incision and correction of the gum; the tissue remains in place, and the procedure helps treat inflammation and improve cleaning access.
- Gingivectomy is the removal of a portion of the gum to make the smile aesthetic and facilitate tooth care.
Thus, the main difference is that a gingivotomy corrects existing tissue, whereas a gingivectomy removes its excess part. Both procedures are performed under local anesthesia and are relatively painless, but they have different indications and goals.
