Dental treatment in sleep, or sedation, is essentially a light, superficial sleep where the patient breathes independently and remains responsive to the doctor. A special drug with a mild sedative effect is administered intravenously. This procedure provides a slight analgesic and relaxation effect, but unlike full general anesthesia, deep muscle relaxation does not occur. Sedation is highly beneficial, for example, for wisdom teeth extraction, as it allows the patient to relax and reduces psycho-emotional stress. The active substance is supplied periodically, alongside local anesthesia; notably, far fewer local anesthetics are required for this manipulation compared to the traditional approach. During dental treatment under general anesthesia, the patient is in a deep sleep state and cannot breathe independently, relying entirely on a ventilator (ICU respirator).

In what cases can you not do without general anesthesia in dentistry?

Sometimes, drug-induced sleep in dentistry (sedation) can be beneficial for children, but it is only suitable for those young patients who can walk into the dentist’s office on their own and freely open their mouth. In other words, if a child already has a psycho-emotional trauma and experiences severe, panicky fear, this technique cannot be used, as the child needs to feel cooperative and comfortable with the doctor and the clinic.

In addition, there is a whole range of other reasons in both pediatric and adult dentistry where general anesthesia may be indicated.

For example, these include:

Severe fear of any dental procedures.
A history of allergic reactions to local anesthetics.
A broad scope of dental intervention.
Wisdom tooth (8th tooth) extraction under general anesthesia when large-scale work is required.
Extraction of multiple teeth or impacted (un-erupted) teeth.
Planning the placement of several implants at once during a single visit.

Patients in whom local anesthesia does not provide the desired effect.

Preparation and conduct of general anesthesia

Wisdom teeth extraction surgery under general anesthesia, as well as any other dental interventions under general anesthesia, require special preparation.

It is necessary to schedule an appointment with an anesthesiologist four to five days before the procedure. You must bring the following examination results with you:

A blood test for clotting time, platelet count, and glucose levels (valid for 14 days).
An electrocardiogram (ECG) or echocardiogram (ECHO) of the heart.
Patients over 50 years of age must have a chest X-ray (fluorography) result (valid for one year).

It is mandatory to have one accompanying person on the day of the surgery. Please bring a change of clothes and shoes. The clothes should be easy to remove, not restrict the body, and should not have complex fasteners or zippers.

Dental treatment under general anesthesia is performed as follows:

The patient lies down in the dental chair, where a mask with an air-Sevorane mixture is placed on them to induce sleep.
After that, a peripheral venous catheter is inserted, through which medications to maintain sleep and provide pain relief will be administered.
Once the patient is completely asleep, tracheal intubation is performed using an endotracheal tube for mechanical ventilation and airway protection.

Throughout the entire surgical intervention, the anesthesiologist stays by the patient’s side, conducting full monitoring of the body’s vital functions using specialized equipment.
When the operation is completed, before the patient wakes up from anesthesia, the breathing tube is removed, and if the situation permits, the venous catheter is removed as well.
Afterward, the patient is transferred to a recovery room, where they will rest in comfortable conditions under medical supervision for some time.

It is also important to know that before general anesthesia, you must not eat food for six hours or drink water for two to three hours. Additionally, the consumption of any alcoholic beverages is prohibited for about two days.
