Dental fluorosis and its causes

Fluorosis is a disease caused by an excess of fluoride in the body, arising from various causes:

High concentration of this element in the drinking water of the patient’s region;
Overuse of fluoride-containing toothpastes and mouthwashes;
An unbalanced diet dominated by fluoride-rich products;
Weakened immune system;Genetic predisposition;
Working in hazardous industries where the concentration of fluoride compounds in the air is high and the element is easily absorbed by the body.

Also quite common is the so-called endemic fluorosis, which is a widespread disease in areas with elevated fluoride levels in the water. If fluorosis treatment is not initiated, besides affecting the teeth, it can lead to disorders of the musculoskeletal system and joints. The disease causes changes in the enamel structure and the appearance of pigmentation—spots and streaks. In advanced stages, fluorosis leads to the destruction of bone tissue. Therefore, it is crucial to start treatment at the first sign of symptoms.

Symptoms

General symptoms of fluorosis include:

Enamel loses its natural shade;
Pigmentation appears on the teeth—yellow, whitish, or brown dots and spots;
Increased enamel wear;
Sensitivity of teeth to temperatures and acidic environments.

In advanced stages, the destruction of hard tissues (bones) occurs.

Stages and Forms of Development

The symptoms directly depend on the stage of fluorosis development — the higher the stage, the more extensive the tooth damage.

According to the WHO classification, several forms of the disease are distinguished:

Very mild: enamel color changes are practically invisible;
Mild: slight appearance of spots and streaks;
Moderate: pigmentation covers no more than 50% of the tooth area;
Medium: accompanied by yellow and brown spots, and a matte appearance of the enamel;
Severe: characterized by distinct pigmentation, wear of affected areas, tooth deformation, and partial enamel destruction.

In addition, the disease is accompanied by more serious consequences, such as musculoskeletal disorders and other health issues.

Types

In total, there are 5 phases of the disease:

Dashed (Striated): thin white lines appear on the teeth, located close to each other;
Speckled: formation of spots with indistinct outlines that merge;
Chalky-speckled: spot outlines become clearer and more noticeable, white often turns to yellow, and the enamel begins to wear down actively;

Erosive: enlargement of the affected areas, accompanied by the destruction of the tooth’s hard tissues;
Destructive: the enamel is almost completely worn away, the dentin is exposed, and the teeth become brittle, leading to their destruction.

In the early stages, fluorosis is practically unnoticeable, but with each subsequent stage, the enlargement of the affected areas is clearly visible. If treatment is not started in the early phases, the patient may lose their teeth.

Fluorosis Treatment

Fluorosis can be diagnosed based on the clinical manifestations of the disease.

Diagnostic methods such as polarized microscopy and microradiography are used for this purpose. Fluorosis treatment is prescribed based on the severity of the tooth surface damage and requires comprehensive therapy.

1. At the First Stage

Traditionally, the treatment course consists of 10 to 15 procedures and includes the following manipulations:

Microabrasion (grinding) of the outer enamel layer;
Treatment with inorganic acids;
Application of potassium gluconate-based preparations;
Electrophoresis — introducing medications into the teeth using electric current;
Photophoresis — treatment with special preparations combined with infrared (IR) radiation.

In the early stages, the affected areas are not severe, and fluorosis is treated through whitening followed by remineralization of the damaged sections. In addition to the classic course, the doctor prescribes dietary supplements that help eliminate fluoride from the body—vitamins, magnesium, calcium, and lactic acid salts. It is also recommended to choose remineralizing toothpastes and avoid those containing fluoride. The sooner you visit our clinic in Kyiv, the higher the chances of successful treatment. 2. At Stages 2–3

Teeth whitening;
Remineralization with Remodent (at least 15–20 procedures);
Prescription of phosphorus-calcium medications and vitamins;
Installation of veneers or composite fillings.

The latter measure is used if the enamel damage is too severe, which is typical for advanced stages of fluorosis. In this case, the doctor and the patient discuss the choice of method for restoring the anatomical shape of the teeth. Besides veneers, composite fillings or crowns may be used. Fluorosis Incidence in Children

Dental fluorosis occurs most frequently in children, as their bodies are more susceptible to various harmful influences and absorb fluoride more easily than adults.

Children find this dental pathology hardest to endure during the mineralization of their permanent teeth. An excess of fluoride during the formation of the skeletal system can lead to disruptions in the enamel structure. It becomes porous, and its appearance changes. Notably, primary (milk) teeth are rarely affected by fluorosis. The disease most commonly manifests during the eruption of permanent teeth. The most frequent cause of fluorosis in children is an elevated concentration of fluoride in drinking water.

Tooth Restoration After Fluorosis. When it is impossible to treat teeth in the final stages of fluorosis, modern dentistry offers patients alternative solutions. In milder forms, aesthetic restoration of the dentition using ceramic veneers or composite filling materials effectively restores the tooth’s shape and a healthy enamel color. In severe cases, orthopedic restoration methods are recommended, such as prosthetics or implantation.

Fluorosis Prevention

In regions where drinking water contains an elevated concentration of fluoride, the following preventive measures should be observed:

Consume filtered or bottled water; alternatively, freeze or boil water before use;
Avoid using fluoride-containing toothpastes and other oral care products;
Reduce the amount of fluoride-rich foods in your diet;
Take vitamins B, D, and C;
Take calcium lactate and calcium gluconate;
Undergo professional dental check-ups 1–2 times a year.

Those with a hereditary predisposition to fluorosis should be especially careful. They are advised to visit the doctor more frequently, reduce dietary fluoride intake, and include more dairy products in their diet, as these help eliminate the element from the body. Workers in hazardous industries are recommended to use personal protective equipment and increase their consumption of dairy products. Since children are most susceptible to fluorosis, they should be examined by a specialist at least once every six months. If a child lives in an area with high fluoride levels in the water, the above preventive measures should be followed, and they should occasionally be taken to other regions for vacation.
