In what cases is dental bone augmentation necessary?

Jawbone volume restoration surgery is most frequently required as a preparatory stage before dental implantation.

Typically, bone grafting is performed to preserve existing teeth and to ensure the reliability of dental prosthetic outcomes. Other indications fall within the scope of maxillofacial surgery.

Bone grafting (osteoplasty) in dentistry is indicated for:

jaw injuries;
jaw lesions resulting from oncological diseases;
defects caused by tooth extractions;

bone resorption due to age;
gunshot wounds;
deficiency of hard tissues for dental prosthetics.

Bone grafting is necessary to restore the volume of tissues that have atrophied due to age-related changes, when degenerative processes begin to occur faster than regeneration. Jawbones can also undergo resorption as a result of a long-term absence of teeth. Without a chewing load, jaw tissues cease to function fully and stop receiving nutrients. Due to metabolic disruptions, the bone atrophies, which then leads to a shift in the dentition. A similar outcome occurs after past periodontitis or infectious diseases.

Furthermore, jawbone grafting may be necessary for aesthetic deficiencies, including those caused by atrophic changes in bone tissue, as well as to eliminate functional disorders.

The surgery is prescribed upon the occurrence of the following defects:

facial contour deformation;
sunken cheeks and lips, wrinkles, sagging skin;
speech sound pronunciation disorders;
bite changes;
severe impairment of chewing function.

With the help of bone grafting, the doctor can ensure the necessary physiological parameters for dental prosthetics (adequate size and density), which positively affects both the aesthetics and stability of the implant.

Contraindications to dental bone augmentation

Jawbone grafting has relatively few contraindications

As a rule, they are common to all types of surgical treatment. However, there are also other absolute and temporary contraindications to bone grafting. The main list includes:

pregnancy and lactation;
leukemia, severe anemia, blood clotting disorders;
neoplasms in the nose and sinuses;
focus of inflammation in the mouth;
acute immunodeficiency;

oncology;
heart problems;
exacerbation of chronic diseases;
osteoporosis, bone repair disorders;
mental illness.

Such interventions are performed only with pain management (general or local anesthesia). Accordingly, intolerance to anesthetics is also considered a contraindication to harvesting or building up the missing jaw bone mass.

Bone grafting methods for implant placement

Bone grafting methods

Several methods have been developed to increase the volume of jaw bone. The doctor chooses the method based on the results of the examination. The main role is played by the degree of bone deficiency and the localization of the defect. To replenish the bone volume in the upper jaw, sinus lifting is used. Bone plastic of the lower jaw is performed using the technology of “wedgement” of the alveolar process, implantation of a donor block, and directed bone tissue repair.

Alveolar process cleft

As a rule, the procedure is prescribed for implantation of several units of future teeth and horizontal bone atrophy. The technology allows you to combine the increase in the volume of hard tissue with the installation of artificial roots. The essence of the method is to saw the alveolar process with its subsequent expansion. Implants are immediately installed in the resulting recess. The remaining space is filled with bone preparation. A collagen membrane is placed on top. The technique allows you to significantly reduce the rehabilitation period.

Guided bone regeneration

Osteoplastics by this method can be combined with implant placement. As a rule, the technology is used for minor and moderate bone resorption in height and width. In the process of jaw bone augmentation, the gums are dissected and exfoliated, and a bone substitute is placed in the defect area. The operated area is covered with a double biomembrane or a film on a titanium mesh. Xenogeneic material is used as a donor block.

Bone block transplant

The operation does not involve combining osteoplasty with implantation. Dental prosthetics will be done after complete tissue restoration. Taking into account individual characteristics, this takes from 6 to 12 months. The technique is used for significant bone tissue deficiency (in height and width). Bone plastic of the lower jaw using this technique takes place in two stages:

obtaining a bone block (chin, wisdom teeth area);
implanting the donor block in place and securing it with titanium screws.

Next, a biomembrane is applied and the gum tissue is returned to its place. The procedure is traumatic, since two areas in the oral cavity require a recovery period at once. However, this option is the most successful in terms of survival, since the patient’s own material is used.

Sinus lift

Sinus lift is a technology for increasing the bone mass of the upper jaw. During the operation, the floor of the maxillary space is shifted upwards. The resulting cavity is filled with bone-substituting material. The intervention is performed in one of two ways:

Materials for jaw bone augmentation

In dental practice, specialized osteoplastic materials are used to build up bone volume.

These materials are manufactured in the form of porous membrane films, granules, and bone blocks. The preparations can be of either synthetic or biological origin. There are several types of bone grafting based on the material used:

autogenous – the patient themselves acts as the donor;
allogeneic – biomaterial from another human;
xenogeneic – biomaterial from an animal donor;
alloplastic – synthetic osteoplastic material (mostly in the form of granules).

Bio-membranes are also classified as materials for bone grafting. These collagen sheets are used to isolate the defect area from the surrounding soft tissues. In this way, the self-resorbing (absorbable) membranes perform a barrier function and stimulate the restoration of the jawbones.

Alternatives to jawbone augmentation

If there is a complete absence of teeth, the “all-on” technology is used — either on four implants (for minor resorption) or on six implants (for a moderate degree of atrophy). This construction is stable and perfectly withstands the load of a full-arch denture. In cases of severe bone deficiency, a similar method is applied — basal implantation.

Additionally, depending on the indications, alternative methods of prosthetics can be utilized:

compression-type dental implants that are placed into deep bone layers;
root-form implants that can be installed at an angle to the dentition or bypassing the maxillary sinuses;
specialized short and narrow implants with increased strength for jawbones with the minimum allowable parameters for implantation.

Another way to avoid jawbone grafting is immediate implant placement. This involves installing the post for the future tooth directly into the socket of a freshly extracted tooth.

Recovery period

After bone grafting, mild swelling is a normal occurrence, and a temporary increase in temperature may also happen. The swelling will continue to increase during the first two days. After 4–5 days, your condition will stabilize. Painful sensations are managed with painkillers prescribed by your doctor. A temporary loss of facial sensation above the surgical area, as well as minor wound discharge (serous fluid), is acceptable. Cold compresses can be applied during the first few days. Bruising may also occur. Throughout the rehabilitation period, patients are advised to follow a gentle diet consisting of soft food that is neither hot nor irritating to the mucous membrane. Procedures that involve heating the wound (baths, saunas, solariums) are strictly prohibited.

You should limit physical activity, avoid air travel, and refrain from positions that promote blood flow to the head. Do not smoke until full healing is complete, and do not consume alcohol. Guard against catching a cold – sneezing and intense nose-blowing can cause the bone graft to shift. Perform oral hygiene carefully, avoiding vigorous rinsing. Be sure to visit your doctor on the scheduled date to monitor the healing process.

You must seek medical assistance immediately if:

the pain does not subside after more than five days;
bleeding does not stop for more than 24 hours;
swelling persists even on the 5th day.
