What is vestibuloplasty?

Vestibuloplastyis a minor surgical procedure aimed at deepening the vestibule of the mouth. This is the space between the lips and the gums or cheeks. It is essentially the “pocket” where we usually place a toothbrush when brushing our teeth.

The main goal is to reduce the risk of gum recession and to strengthen the gums.
  • It acts as a “helper” in prosthodontics or implantology.
  • It improves speech and corrects certain speech defects.
  • It can be performed using various methods, such as a laser or a scalpel.
  • The procedure takes only 20–40 minutes, and the recovery period lasts 2–4 weeks.

In dentistry, vestibuloplasty is actually considered a basic and straightforward procedure. For instance, unlike dental implantation, it is less invasive and is performed much faster. Vestibuloplasty strengthens the gums, makes the smile look healthier, and helps ensure teeth stay firmly in place, whether with prosthetics or without.

Furthermore, increasing the depth of this vestibule (pocket) significantly minimizes tooth mobility and even prevents tooth loss. It also resolves potential issues related to the placement of dental implants. Before installing orthodontic appliances, it helps create sufficient space and a solid foundation for them. Additionally, it eases overall daily oral hygiene.

Sometimes, it is also used in pediatric dentistry—specifically for a short frenulum or an oral anatomy that interferes with proper speech or bite development. There is no pain during the procedure, and any discomfort is kept to a minimum.

How to understand that it is necessary to increase the depth of the vestibule of the oral cavity

A person can notice the need to increase the depth of the oral vestibule on their own if the gums seem to “recede” and the teeth become more visible. In some cases, the roots of the teeth may already be exposed. Additionally, if discomfort occurs during brushing, this can also be an early sign of a vestibule problem. However, the final decision is, of course, made by a qualified dentist after a patient examination and a complete diagnosis.

There are many symptoms and conditions that justify deepening the vestibule. In other words, common indications for vestibuloplasty include:

  • The formation of a “pocket” between the teeth and gums, along with insufficient vestibule depth.
  • A short frenulum that pulls on the gums.
  • Speech difficulties and a misaligned bite (malocclusion).
  • Increased sensitivity at the base of the tooth.
  • Excessive plaque accumulation despite proper hygiene—especially at the top or bottom of the teeth.
  • Poor retention or the inability to secure dentures and implants.
  • Frequent gum inflammation and tooth mobility.

Types of vestibuloplasty

There are different types of vestibuloplasty. They mainly differ by the method used—specifically, how invasive or minimally invasive they are. The classic option involves using a scalpel to detach the necessary tissues, followed by suturing. Of course, the entire procedure is performed under local anesthesia. Another frequently used modern option is the laser method. It is virtually bloodless, sometimes requires no stitches at all, and allows for a faster recovery

Generally, vestibuloplasty involves the following types:

  • Classic Scalpel Method: An affordable and widely used option performed with a scalpel. After making a small incision in the gum, the necessary tissue is detached to increase the depth of the vestibule. Healing takes longer compared to some other types of vestibuloplasty, but it is chosen for complex cases (such as severe gum recession or crucial preparation for dental implants).
  • Laser Method: A modern and highly popular type of vestibuloplasty that is much less invasive. It usually does not require sutures and ensures rapid tissue recovery. Deepening the oral vestibule this way is ideal for minor defects and a smaller overall volume of work. It is considered the gold standard today.
  • Ultrasonic Method: Used less frequently, as it requires specialized equipment and is not suitable for all cases (for instance, large treatment areas). This method uses a scaler (a special ultrasonic device) that gently incises and detaches the gum tissue. However, it causes less trauma than a scalpel.
  • Electrosurgical Method: Another less common type of procedure, though it is also minimally invasive. It is performed through blood vessel coagulation (sealing) and is most often used only in small areas of the oral cavity, as well as for patients with thinner gums. However, it may cause more discomfort during the recovery period compared to the laser or even the scalpel method.

Recovery period and possible complications

The recovery period after an operation such as vestibule plastic surgery is somewhere between 2 and 4 weeks. But the primary healing itself occurs within the first 7-14 days. How long vestibuloplasty heals depends primarily on the type of operation and the total volume of interventions. It also depends on the individual characteristics of the patient – his initial condition of the gums, the speed of tissue recovery, etc.

To ensure that recovery after vestibuloplasty is as safe and fast as possible, doctors recommend the following:

  • Do not traumatize or touch the gums with your tongue or hands.
  • Avoid hard, crunchy, and excessively hot foods and beverages.
  • Rinse your mouth with an antiseptic solution if prescribed by your dentist.
  • Brush your teeth gently, preferably using a soft-bristled toothbrush.
  • Take painkillers or anti-inflammatory medications if necessary and prescribed by your doctor.
  • Temporarily avoid alcohol and smoking.
  • Strictly follow all other instructions provided by your dentist.

Failure to follow these recommendations can lead to complications after vestibuloplasty, such as bleeding and swelling, poor scarring, a recurrent loss of vestibule depth, and sometimes even infection.
