What Are Tooth Cracks and What Do They Look Like?

A tooth crack is a break in the continuity of the tooth — typically appearing as a fine line visible on the enamel surface. Cracks can also extend into the dentin, which is the inner layer of the tooth beneath the enamel. They develop as a result of enamel wear or excessive force placed on the tooth. Front teeth are the most commonly affected, as their enamel is thinner and therefore more vulnerable.

The main signs that a tooth has cracked include:

  • Fine vertical lines on the tooth surface
  • Discoloration of the tooth
  • Localized sensitivity (and occasionally pain)
  • A rough or uneven texture on the tooth surface

Telling a tooth crack apart from cavities is actually quite straightforward. A crack may cause the tooth to look yellowed or dull — and only if the dentin has been reached. Tooth decay, by contrast, presents as either a visible pit or a noticeably dark spot. As for distinguishing between a crack in the enamel versus a deeper fracture, the presence of pain is the key indicator. When only the outer enamel layer is affected, there is almost never any pain. However, if the crack has penetrated deeper into the dentin or pulp, the pain can be quite pronounced.

Types of Tooth Enamel Cracks

Tooth cracks are generally classified into two main categories:

Surface cracks. These are confined to the enamel layer and are painless. They may be visible or barely noticeable to the eye. In most cases, surface cracks cause no discomfort beyond a cosmetic concern and do not require urgent treatment.

Deep cracks. These extend into the dentin and sometimes reach the pulp. They are most commonly associated with significant sensitivity and pain, and typically require dental treatment — including, in many cases, a filling or more involved restoration.

Tooth cracks in children and adults differ due to the distinct nature of dental development at different life stages. Children’s enamel is thinner and their teeth are still forming, making them more susceptible to cracking. In adults, teeth are more prone to natural wear and gradual deterioration over time.

When a child’s tooth cracks, it is often the result of trauma, falls, poor oral hygiene, or an unbalanced diet. When an adult’s tooth cracks, stress or bruxism (involuntary teeth grinding) are frequent culprits.

What to Do If Your Tooth Has Cracked

The first thing to do when a tooth cracks is to avoid anything that could make it worse — don’t chew on that side (especially hard foods), and avoid very cold or very hot food and drinks. Once you’ve assessed the situation, book an appointment with your dentist as soon as possible. The treatment your dentist recommends will depend on the condition of the cracked tooth. Options include:

As preventive and strengthening measures, fluoride treatment (to seal microcracks and protect the enamel) and remineralization therapy (to restore minerals to the enamel) may be applied.

Aesthetic restoration with veneers or crowns. Both veneers and crowns can be used to improve the appearance of a cracked tooth and protect its structural integrity. These restorations rebuild the shape of the tooth while shielding it from further damage.

Filling. Often necessary for deeper cracks that, if left untreated, could eventually lead to decay.

It’s also worth knowing what to do if a tooth that already has a filling develops a crack. In this case, prompt attention is equally important — to prevent further breakdown of the tooth and avoid potential complications. What Happens If You Leave a Cracked Tooth Untreated?

It’s worth understanding that existing cracks do not heal on their own — teeth cannot repair cracks by themselves. However, leaving cracks untreated can lead to a range of complications.
