What is a lateral cephalogram?

TRH (teleradiography) in the lateral projection is an X-ray picture of the skull from the side, which is used in dentistry and orthodontics. It helps to assess the bite, the position of the teeth and jaws, the proportions of the face and to plan orthodontic treatment.

How long does it take to perform a cephalometric analysis (TRG)?

Teleradiography requires no special preparation, which saves time. The procedure itself takes about ten minutes, and the radiologist requires additional time to interpret the resulting images.

How is teleradiography performed?

The cephalometric imaging procedure does not take long. The patient is asked to stand or sit upright. Plastic ear plugs are inserted into the ear canals, and a brace is placed against the bridge of the nose to help keep the head motionless. If the doctor has ordered a contrast scan, a special paste is applied to specific areas of the patient’s face. The results are provided once the procedure is complete. However, one cannot simply look at the image immediately; before it reaches the specialist, a radiologist must analyze, measure, and process the data to determine whether the cephalometric findings are within the normal range.

Indications and contraindications for the procedure

Indications for the procedure

Given the specifics of the dental TRG procedure, it is prescribed based on certain indications, such as:

To determine the strategy and methodology for further therapy.
Inflammatory lesions of unclear origin in the upper and lower jaw regions.
Trauma or pathologies affecting the dentofacial apparatus and the facial region as a whole.

When a pathological process in the jaw joint is suspected.
When it is necessary to monitor the growth direction of unerupted teeth.

Contraindications

In its spectrum, teleroentgenographic examination has ten times less radiation than, for example, fluorography. But, although it is practically harmless, it still has a number of contraindications, in particular, such as: pregnancy, breastfeeding, children under the age of 6 can have TRG (only in exceptional cases), bleeding, serious health conditions, undergoing radiation therapy.

At certain stages of dental treatment, a doctor may prescribe TRH of the jaw more than once. This is, as a rule, at the preparatory stage, later in the treatment process and at the final phase of the therapy for accurate control of the results. It is important to understand that any radiation exposure received by the patient during the year is always added up. If there was a fact of x-ray examination for non-dental purposes, the doctor must be notified, if the information was not entered in the medical record.

Cephalometric landmarks

The doctor requires calculations to plan the treatment.

Previously, X-ray images were printed out, and the doctor had to identify key points on them and then use tools—such as a ruler, protractor, tracing paper, and pencil—to calculate angles, size ratios, and distances. This method did not yield absolute precision, as even the thickness of the pencil line could introduce unwanted inaccuracies. However, the advent of teleradiography (TRG) made it possible to automate this stage. Data from the teleradiograph is uploaded into specialized software capable of plotting over 75 points and connecting them into diagrams, thereby providing the doctor with access to all the necessary results.

Types of Teleradiographic Images

As previously mentioned, a teleradiograph is a key step in diagnosing various dental pathologies, as it allows for X-ray imaging of the head in both lateral and frontal projections. The lateral view is essential for identifying jaw growth pathologies and bite anomalies, as well as for determining the appropriate treatment plan. Analyzing a lateral teleradiograph makes it easy to assess the inclination angles of the jaws and teeth and to detect malocclusion in children at an early stage, enabling timely intervention. Meanwhile, a frontal view of the skull reveals facial proportion irregularities and the general condition of the upper and lower jaws, bone tissue, and teeth; it also helps identify potential inflammatory processes in the nasal sinuses and nasal septum pathologies.
