Causes of TMJ Osteoarthritis

TMJ osteoarthritis is fundamentally traumatic in origin — it arises when the joint surface sustains damage, developing erosions and microcracks over time. This trauma doesn’t have to come from a direct blow or impact; it can also be triggered by a range of other conditions and dysfunctions, including:

  • Traumatic arthritis
  • Injuries caused by abnormal jaw or bite development, prosthetic errors, sudden or forceful jaw movements, or birth trauma
  • Complete tooth loss in both jaws
  • Injection of certain medications directly into the joint cavity

Additional potential causes include infection, subsequent trauma, metabolic disorders, and atherosclerosis of the terminal vascular branches. The risk of developing the condition also increases during the perimenopausal [literally “climacteric”] period, which is often accompanied by endocrine imbalances. TMJ osteoarthritis can affect patients of any age — even newborns can develop the condition as a result of birth trauma. In older adults, the likelihood of involutive (age-related) osteoarthritis rises due to the natural decline in the body’s ability to regenerate tissue.

How Is TMJ Osteoarthritis Treated?

Treatment for TMJ osteoarthritis begins only after the diagnosis has been confirmed through proper testing. This requires a complete blood count and biochemical blood panel, X-rays taken from two projections, and a CT scan or MRI — in some cases, an ultrasound may also be needed.

It’s important to understand that self-diagnosing TMJ osteoarthritis is not possible. Even mild or early symptoms warrant a visit to a specialist. TMJ osteoarthritis is always treated using a comprehensive, multi-modal approach, regardless of how advanced the condition is.

In the early stages, the disease may not produce obvious symptoms, though signs of dysfunction can begin to appear — so the treatment focus at this stage centers on myogymnastics (targeted jaw muscle exercises) and physiotherapy aimed at restoring normal jaw function. This is followed by correction of the condylar head position, dental arch alignment, and bite. Concurrently, medications are used to enhance joint metabolism and support tissue regeneration.

In cases where ossification (bony fusion) has occurred, or where conservative methods have proven insufficiently effective, surgical intervention may be required — involving replacement of the joint or a portion of it with a transplant or implant.
