Odontogenic sinusitis is an inflammatory process of the mucous membrane inside the paired paranasal sinuses (maxillary cavity).

What are the symptoms of odontogenic sinusitis?

Odontogenic sinusitis is manifested by nasal congestion on one side, purulent or mucopurulent discharge, pain in the upper jaw or teeth, swelling of the cheek, bad breath, and general malaise. The pain may worsen when the head is tilted.

What are the causes of odontogenic sinusitis?

Odontogenic sinusitis occurs due to an infection spreading from diseased upper teeth into the maxillary sinus. The main causes are advanced caries, pulpitis, periodontitis, dental cysts, complications after tooth extraction or treatment, as well as poor-quality fillings or implants.

What does the treatment of odontogenic sinusitis include?

Symptoms of sinusitis

Symptoms of maxillary sinusitis (highmoritis) depend on the duration of the disease, individual characteristics of the organism, and other factors.

However, general symptoms can be identified, some of which manifest in the area of the maxillary sinus. Each of them is a reason to seek medical assistance:

Heaviness
Tenderness (painfulness)
Swelling (puffiness)
Purulent discharge
Prolonged runny nose
Impaired sense of smell
Foreign odors in the nose
Headaches
Difficulty breathing

Acute sinusitis is accompanied by a severe deterioration in general well-being, drowsiness, and a fever of up to 40 degrees Celsius. In its chronic form, sinusitis may occur periodically and then resolve on its own.

Causes of odontogenic (dental) sinusitis

Odontogenic sinusitis occurs due to the penetration of pathogenic flora into the maxillary sinus. This can be caused by pathologies of the upper dentition due to periodontitis, tooth extraction, or root canal filling, when restorative material can enter the sinus cavity.

Please note, sinusitis does not clear up on its own; it requires treatment.

Complications of sinusitis

Sinusitis is dangerous by spreading the infection.

As a result of infection of neighboring tissues, a cyst, meningitis or purulent lesions of soft tissues may occur. Patients prone to allergic reactions may experience their deterioration. The most dangerous complications are cancer of the nasal sinuses and pansinusitis, characterized by inflammation of the entire nasal cavity, the cause of which is often chronic sinusitis.

How is sinusitis treated?

Treatment begins with establishing the root cause that triggered the sinusitis.

If cysts grown into the maxillary sinus are observed, surgical intervention via maxillary sinusotomy (antrotomy/maxillary sinus surgery) is required, during which the altered mucous membrane is removed. Maxillary sinusotomy is performed in dental surgery. In cases where a tooth is identified as the cause of sinusitis, it must be extracted. Afterward, through the socket formed after extraction, the surgeon removes the excess mucous membrane in the maxillary sinus. In situations where the tooth has already been removed previously, an ultrasonic scalpel is used. During the operation, an incision is made inside the oral cavity above the teeth, through which the maxillary sinus is cleared. Surgical treatment of sinusitis is performed under local anesthesia, and the invasiveness is minimal. Immediately after the surgery, headaches disappear, breathing and the sense of smell improve, and foreign odors and heaviness vanish. Conservative treatment is of low efficacy and is rarely used, only for the purpose of temporary relief.

Rehabilitation and Recovery

Recovery after sinusitis is quite fast and not demanding for the patient. Follow the doctor’s recommendations.

A tamponade may be required for the operated area during the first 24 hours after surgery.
Taking pain relievers.
Thorough cleaning of the oral cavity.
