What is periostitis?

Periostitis is an inflammation of the periosteum—the tissue that covers the jawbones and the alveolar processes where the teeth are located.

This usually occurs as a result of an infection that can arise due to dental caries, trauma, gum disease, or other causes. Localization of the disease in the lower jaw occurs approximately 1.5 to 2 times more frequently than in the upper jaw. The course of jaw periostitis has a specific local and general clinical picture, and with timely treatment, the inflammatory process can be reversible. However, as the disease progresses, the risk of developing severe purulent complications increases.

Symptoms of periostitis

Jaw periostitis is accompanied by a number of characteristic symptoms:

Swelling of the gums in the area of the affected jaw.
Pain, which can be either dull and aching or sharp and unbearable upon touching the affected area or during percussion.
Elevated body temperature, a feeling of general weakness, headaches, and other manifestations of body intoxication.

In chronic periostitis, a change in the shape and size of the affected jaw, the development of cystic formations, and tooth deformities may be observed.

If such symptoms appear, it is necessary to immediately consult a doctor for diagnostics and treatment prescription.

Types of periostitis

There are numerous types and subtypes of periostitis, for which appropriate classifications have been developed.

The varieties of periostitis depend on the pathway through which the infection enters the periosteum and include the following types:

Odontogenic periostitis (develops as a result of dental diseases);
Hematogenous (occurs when the infection spreads through the bloodstream from other affected organs);
Lymphogenous (the infection enters the periosteum through the body’s lymphatic system);
Traumatic (arises due to mechanical injuries, such as a jaw fracture).

Periostitis can occur in either an acute or chronic form. Acute periostitis is characterized by a rapid onset of symptoms and pronounced pain. In this case, swelling, hyperemia, and painful sensations in the affected area may appear. The patient may also experience elevated body temperature, weakness, headaches, and other signs of intoxication. Chronic periostitis develops slowly and can remain asymptomatic for a long time. A characteristic feature is the presence of persistent, low-intensity pain at the site of the lesion. With a prolonged course of periostitis, the formation of a granuloma—a closed purulent focus surrounded by connective tissue—is possible.

Periostitis in children

Jaw periostitis in children has its own specific features.

First and foremost, unlike adults, children are more likely to develop odontogenic periostitis, which is linked to dental infections. Furthermore, during childhood, periostitis develops more rapidly and follows a more severe course. Children experience toothaches, severe pain when biting or chewing food, as well as swelling and redness of the gums. They may also experience difficulties opening their mouth or bringing their teeth together. In the case of acute periostitis, treatment usually begins with the prescription of antibacterial drugs aimed at fighting the infection. If purulent foci are present, incision and drainage are performed. Analgesics and anti-inflammatory drugs are used to relieve swelling and pain. Chronic periostitis in children requires longer and more comprehensive treatment. As a rule, physiotherapy methods such as ultrasound, laser therapy, and magnetotherapy are utilized to stimulate tissue healing and reduce inflammation.

An important aspect of treating periostitis in children is disease prevention. To achieve this, it is necessary to monitor oral hygiene, conduct regular dental check-ups, and treat dental diseases in a timely manner. It is also essential to ensure a healthy diet for the child and boost their immunity.
