Frequently Asked Questions:

Is it safe to have dental treatment during pregnancy?

Dental treatment during pregnancy is not only possible but highly recommended. In the absence of specific contraindications, timely dental care helps prevent infections that could affect both the mother’s health and the baby’s well-being.

Contraindications for dental procedures during pregnancy

Contraindications for treating teeth and gums in expectant mothers include the following:

  • Severe morning sickness (toxicoses);
  • The first and third trimesters (early and late stages of pregnancy);
  • Exacerbation of chronic diseases;
  • Risk of pregnancy complications or miscarriage;
  • Individual health factors during the gestation period.

The selection of a method to address any dental issue during pregnancy is handled on an individual basis. The dentist takes into account the stage of pregnancy, the nature and severity of the condition, and the patient’s medical history.

Anesthesia During Dental Treatment for Pregnant Patients

During dental procedures during pregnancy, medications are used that have a minimal effect on the blood vessels and cannot cross the placental barrier. In modern dentistry, the preferred choice for pain relief is articaine-based anesthetics. These are non-toxic, do not cause allergies or other adverse reactions, and effectively eliminate discomfort during procedures. General anesthesia is not used for dental treatment during pregnancy.

What to do if a pregnant woman has a toothache?

The first and most important thing a pregnant woman can do for a toothache is to consult a dentist. Only a doctor can select a pain reliever that is safe for her specific case.

Is professional teeth cleaning allowed during pregnancy?

Teeth cleaning during pregnancy is permitted and often even recommended. It effectively reduces the risk of developing cavities, gingivitis, and other similar oral health issues.

Can pregnant women have dental treatment with local anesthesia (injections)?

Yes, dental treatment with local anesthesia is allowed for pregnant women, but the specific medication must be selected exclusively by a doctor. Typically, epinephrine-free (adrenaline-free) anesthetics are used, as they are safe and permitted in most cases during pregnancy.

Is it possible to have dental treatment in the third trimester?

Dental treatment in the third trimester is possible, but only if there is an urgent need. It is recommended to avoid long procedures that can be postponed without harm to the woman’s health during this period.

Can teeth be filled during pregnancy?

Whether a pregnant woman can have a tooth filling is determined by specific medical indications. If indicated, there are no issues. Filling a tooth is considered one of the safest procedures for pregnant patients.

Is it safe to have dental treatment during pregnancy?

Dental treatment during pregnancy is possible. Moreover, it is necessary if there are clear medical indications. Modern dental materials and anesthetics are largely safe and approved for use during pregnancy. Treatment is permitted in all three trimesters, but always requires a consultation with a doctor to account for the woman’s specific health condition. The second trimester is considered the safest time for dental procedures.

Procedures that are almost always permitted during pregnancy:

  • Safe pain relief (anesthesia);
  • Professional teeth cleaning;
  • Fillings and cavity treatment;
  • Gum treatment;
  • Tooth extraction (only in cases of absolute necessity).

Regarding dental implants, prosthetics, and whitening, doctors generally advise postponing these until after childbirth unless there is an urgent need. With the right approach, dental intervention during pregnancy poses no risk. However, it is essential to use correctly selected medications and dosages and to avoid X-rays whenever possible.

Avoiding treatment for dental or gum problems during pregnancy entirely is also incorrect. Constant discomfort and stress from pain or inflammation can also be harmful. For example, if you experience acute toothache during pregnancy, you should see a dentist as soon as possible. The doctor will select the appropriate anesthesia and relieve your condition!

Ultimately, regarding any question about whether it is possible to treat, extract, or restore teeth during pregnancy, the decision is made by a doctor. The best approach is coordinated care between the dentist and the obstetrician-gynecologist supervising the pregnancy.

Diagnostics

Dental Diagnostics During Pregnancy. Tooth X-ray During Pregnancy.

In addition to visual inspection, the dentist may refer the patient for additional examination. Most often, this concerns cases of the possibility of preserving or the necessity of removing a diseased tooth, as well as in order to obtain the most accurate picture for endodontic treatment. Visual inspection does not allow diagnosing a cyst and hidden carious lesions in a pregnant woman. X-rays are prescribed very rarely and only in the second trimester. At the same time, the woman’s abdominal and pelvic area is covered with a special lead apron, which significantly reduces the risk of harmful effects on “danger zones.” In the first and third trimesters, a radiovisiograph is used in modern dentistry to establish an accurate diagnosis. This device is characterized by a low level of radiation. The image is taken using a special small sensor that is applied directly to the tooth.

Most Common Dental Diseases in Pregnant Women

Due to intense calcium loss, changes in hormonal levels, and the weakening of the immune system during the gestation period, the following dental problems may arise:

  1. Gingivitis. Gums during pregnancy are more vulnerable to inflammatory processes and bleeding. It is important to maintain sufficient oral hygiene and use mouthwashes with bactericidal properties.
  2. Caries. The cause of carious lesions in future mothers is the natural loss of calcium and fluoride. In addition, hidden caries could have started developing even before the onset of pregnancy. It is necessary to get rid of such an unpleasant pathology. In women in position, caries progresses rapidly.
  3. Splitting or loss of a filling. Pathogenic microbes penetrate an open cavity very quickly. It is necessary to urgently consult a doctor.
  4. Periodontitis. If such a disease is not treated, the teeth become loose and fistulas form. The infection spreads quickly throughout the body and can cause infection of the fetus and miscarriage.

  1. Periodontal disease. Due to the exposure of the neck, teeth become mobile and there is a risk of losing them.
  2. Pulpitis in pregnant women is accompanied by an infection that can also penetrate the placenta.
  3. Also, due to the weakening of the immune system, women in position are often prone to stomatitis. Such a disease can transform into a chronic form and cause thrush.
  4. Flux (Gum boil) is another of the most dangerous pathologies during pregnancy. Periostitis causes general intoxication of the body and is accompanied by acute pain. The breakthrough of the purulent area into surrounding tissues is dangerous for the life and health of the mother and future child. In such a case, tooth extraction and subsequent antibiotic therapy may be recommended.

Do not neglect the treatment of the disease — contact a doctor immediately if something bothers you.

Nuances of Dental Treatment During Pregnancy

There are, of course, nuances to dental treatment during pregnancy. As with any treatment in general. However, all dental manipulations are carried out with great caution, taking into account the woman’s condition.

The main nuances here are:

  • Treatment timing — it is better to do this specifically in the 2nd trimester.
  • Pain-relieving anesthesia — which painkiller can be used for pregnant women is selected only by a doctor. It is important that there is no adrenaline in the composition.
  • Other medicines and drugs — they must all be safe, even those that a woman takes independently at home. For pain, anti-inflammatory, antibiotics, antiseptic drugs — all of this is coordinated with doctors.
  • Radiography — it is better to avoid it entirely. It is done only in cases of extreme urgent necessity. Although, it should be noted that modern X-ray machines have a fairly low level of radiation. But at the same time, everything must also be only with the permission of doctors.

It is also important to generally avoid stressful and long-term dental procedures during pregnancy. If treatment is still very necessary, it is better to divide it into several stages. It is also important to monitor your condition — both before dental manipulations, during them, and after.

Limitations and Contraindications

Some dental procedures are limited or completely contraindicated during pregnancy because the body is overly sensitive during this period. Everything changes — immunity weakens, the oral mucosa becomes more vulnerable, and the hormonal background is also altered. Both the woman and the baby inside can react to any stress, even minimal. Dental operations, jaw radiography, strong painkillers — all of this can cause uterine hypertonicity. That is why dentists weigh all the nuances to minimize risks as much as possible. Or, they advise postponing treatment and any dental manipulations for later, after childbirth has passed.

For example, whether pregnant women can have teeth pulled is one of the most important questions. Tooth extraction is considered a risk because it is a surgical intervention. But there is also something else.

So, all the points characterized as dental limitations and contraindications during pregnancy, which at the very least definitely require a thorough assessment by doctors for permission:

  • Diagnostics and pain relief — general anesthesia and sedation, X-rays, anesthesia with adrenaline, some anti-inflammatory drugs, and antibiotics are not recommended.
  • Surgery — tooth extraction, orthognathic operations, and implantation are not recommended.
  • Invasive, aggressive treatment methods — chemical treatment of the gums and ultrasonic hygiene are not recommended.
  • Aesthetic procedures — teeth whitening (both with drugs and laser), installation of braces, and veneers are not recommended.

Treatment in Different Trimesters

Treatment in different trimesters is, of course, different — both in terms of permissions and in terms of the use of certain drugs and manipulations. If teeth ache during pregnancy and it is unbearable or could potentially lead to some complications — treatment is necessary. However, what is not allowed in the first trimester, for example, is allowed in the second or third. Therefore, it is worth knowing these differences and always considering them during pregnancy.

Next, we will specifically analyze all questions regarding different trimesters. Namely — what pregnant women can take for toothache, whether it is possible to pull teeth during pregnancy, etc.

First Trimester

Dental treatment in the first trimester is only as needed. This is the most delicate period because the organs and systems of the fetus are still forming, and stress or medications can affect this development. It is recommended to refrain from planned procedures that are not urgent. Emergency treatment here is usually allowed only in cases of very severe pain, swelling, or purulent inflammation. The main risks in the first trimester are infections, anesthesia, and the influence of medications, as well as stress from the manipulations themselves. Tooth extraction is also highly undesirable. For toothache, it is recommended to either apply cold or rinse with soda/chamomile. If any dental manipulations are still performed, in addition to the dentist’s permission, the permission of the gynecologist managing the pregnancy is also required.

Second Trimester

Dental treatment in the second trimester is possible; most procedures are even allowed based on indications. Especially if the treatment is truly necessary and all nuances or significant contraindications are taken into account. The organs of the fetus are already formed, as is known, so it will be slightly safer for the patient to sit in the dentist’s chair. Professional hygiene, filling and treatment of caries, treatment of periodontitis, medicinal elimination of toothache, and sometimes even certain surgical interventions (for example, tooth extraction based on clear indications) — all of this is largely possible. Anesthesia here is still without adrenaline, and radiography is only in case of acute need and with special protection for the body. Paracetamol is already allowed for toothache.

Third Trimester

Dental treatment in the third trimester is only in case of urgent necessity and taking into account the woman’s well-being. If there is treatment, it will be with great caution because, firstly, a woman during this period already gets tired significantly more (lying down for a long time, enduring even minor discomfort). Secondly, the uterus already increases significantly, and specifically a long position on the back can adversely affect large vessels. This, in turn, can lead to nausea, dizziness, and sometimes even loss of consciousness. Only urgent emergency actions are recommended, and preferably in a more semi-sitting position. If possible, use alternative options that can sort of postpone full treatment until the period after childbirth. For example, a temporary filling instead of a permanent one. And how to treat gum inflammation in pregnant women during this period? Thanks to allowed medications, but without deep intervention (such as in endodontic treatment). For toothache, it is also better to use only local remedies coordinated with a doctor.

Recommendations for Dental Care During Pregnancy

Recommendations for dental care during pregnancy — specifically, how to relieve toothache at home for pregnant women or how often to have professional cleaning — are provided by a dentist. It often happens that treatment for gingivitis in pregnant women was performed, but it returned due to hormonal changes (gums become more sensitive and often swell for no apparent reason).

So, recommendations for home dental care during pregnancy:

  • If there is gum inflammation — use only a soft toothbrush, be careful when using floss, rinse with alcohol-free mouthwashes or chamomile, and reduce the consumption of sugar and hard foods.
  • If there is bleeding of the gums — continue to brush your teeth, but use an ultra-soft brush. Also, avoid floss and toothpicks, and rinse with an antiseptic solution.
  • If there is plaque and tartar — use a toothpaste selected with a doctor, an additional interdental brush (proximal brush), and regular tongue hygiene.
  • If there is increased tooth sensitivity — avoid very cold or hot, as well as sweet things. Also, use a paste specifically for sensitive teeth selected with a doctor, a soft brush, and brushing without pressure.
  • If there is initial caries — enhanced home hygiene, avoid sweets and frequent snacking in general.

In general, during pregnancy, it is important to pay special attention to hygiene, but also to postpone stressful dental manipulations that can be delayed. Safe drugs for toothache, the absence of aggressive components in any products, regular monitoring of teeth and gums, and timely preventive cleanings — all of this should be present. But the main thing: coordinate everything with both the dentist and the gynecologist.
