What is phlegmon?

In most cases, the pathology has an odontogenic origin, however, it is also possible for it to occur in purulent processes in the throat and tonsils, for example, in angina. One of the main characteristics of paramaxillary phlegmon is the formation of purulent foci, in which there is no clear boundary between the affected and healthy tissue. In the absence of effective therapy, the pathology can lead to serious complications, such as sepsis or the spread of infection to the brain. Therefore, it is important to consult a dentist at the first signs of the disease and receive appropriate treatment. Phlegmon of the face manifests itself in the form of a vague, painful tumor-like formation, in which the skin and mucous membrane covering it swell, redden and become inflamed. In the absence of treatment, the tumor increases in size and becomes denser. In the central areas of inflammation, where purulent tissue melting is most pronounced, a fluctuation zone appears. If purulent inflammation is located in the deep layers, local symptoms may be less pronounced.

Why does phlegmon occur?

Facial phlegmon occurs due to an infection that spreads through the soft tissues.

This infection can be caused by staphylococci and other pyogenic (pus-forming) microorganisms. Some of the most common causes of odontogenic (tooth-related) phlegmon include:

  • Tooth or gum infections: Dental caries, tooth abscesses, periodontitis, and gum diseases such as gingivitis and periodontitis can lead to an infection that spreads to the soft tissues of the face.
  • Trauma: Traumatic injury to the facial soft tissues, such as a blow, concussion, or bone fracture, can lead to infection.
  • Surgical procedures: Pathological processes can develop after surgical interventions, such as tooth extractions or facial plastic surgery.
  • Weakened immunity: Individuals with a compromised immune system (the elderly, children, or patients undergoing chemotherapy or taking immunosuppressants) are more susceptible to developing infections and facial phlegmon.
  • Risk factors such as diabetes, alcohol abuse, and smoking increase the likelihood of developing phlegmon in the facial region.

Symptoms of Phlegmon

Symptoms of facial phlegmon can vary depending on the location of the infection and its spread. The most common clinical signs include:

  • Swelling of the facial soft tissues;
  • Pain in the affected area: the pain can be sharp or dull, and may intensify when touched;
  • Hyperemia (redness) of the skin;
  • Increase in localized and general body temperature (fever);
  • Headaches;
  • Feeling of weakness and increased fatigue;
  • Swelling of the eyelids;
  • Difficulty breathing or swallowing if the infection spreads to the throat.

Types of phlegmon

The classification of facial phlegmons is based on the anatomical location of the infectious focus and the extent of its spread.

Some of the common types of this purulent process include:

  • Phlegmon of the upper lip and nose;
  • Subcutaneous facial phlegmon (the infection spreads beneath the facial skin but does not affect deep tissues);
  • Deep facial tissue phlegmon (the inflammatory process affects deep facial structures, such as muscles and bones);
  • Buccal/Cheek phlegmon;
  • Submandibular phlegmon (the infection specifically affects the submandibular region, including the salivary glands);
  • Pharyngeal phlegmon (the infection spreads to the throat and can lead to breathing difficulties).

Each of these types of facial phlegmon requires an individualized approach to treatment.

Additionally, there are four stages of the disease’s progression:

Stage I: Serous (a small swelling forms on the skin, caused by fluid accumulation);

Stage II: Purulent (pus forms under the skin, causing intense pain and pronounced hyperemia in the affected area; body temperature rises during this stage);

Stage III: Suppurative-destructive (tissue destruction occurs, accompanied by a foul odor and systemic intoxication of the body);

Stage IV: Necrotic (necrosis or death of the inflamed tissues).

Treatment methods

Treatment of facial phlegmon may include several methods depending on the severity of the infection and the location of its spread.

The primary methods for treating facial phlegmon include:

  1. Antibiotics: Medications are used to eradicate the bacterial infection, with broad-spectrum agents being prescribed most frequently.
  2. Anti-inflammatory drugs: Non-steroidal anti-inflammatory drugs (NSAIDs) are aimed at eliminating swelling and reducing pain.
  3. Oxygen therapy: If the infection spreads to the throat, oxygen therapy may be required.
  1. Surgical intervention: In some cases, a surgical procedure is performed to incise the phlegmon, drain the purulent content, and flush the cavity, which helps eliminate the infection.
  2. Physical therapy: Physiotherapeutic procedures can help relieve pain, reduce swelling, and decrease inflammation.
  3. Pain relievers (analgesics).

How is an abscess or phlegmon of the maxillofacial region treated?

Comprehensive treatment is required, consisting of several components:

  • Elimination of the root cause of the disease (tooth extraction);
  • Incision and drainage of the abscess (phlegmon);
  • Antibacterial therapy (antibiotics);
  • Targeting the mechanism of disease development (pathogenesis).

Stages of the Surgery

  1. Antiseptic treatment of the area affected by the abscess;
  2. Administration of anesthesia for pain relief prior to surgical intervention (local infiltration or general anesthesia);
  3. Incision of the abscess focus using either an external or intraoral approach, depending on the location;
  4. Evacuation of pus from the affected space;
  5. Revision of the cavity to determine the extent of tissue damage (digital or instrumental examination);
  1. Opening of sinuses and pockets;
  2. Debridement (removal) of necrotic tissues;
  3. Antiseptic irrigation of the cavity;
  4. Insertion of drainage devices (gauze swab, drainage tube);
  5. Application of standard protocols for treating purulent wounds.

Rehabilitation and recovery

The postoperative period plays a significant role.
