What is osteoplasty?

Bone grafting (osteoplasty) is a reliable and effective way to build up the necessary volume of jaw hard tissue.

This procedure significantly increases the reliability of the implant root’s osseointegration: for dental prosthetics to be successful, the jawbone must meet specific parameters (width, height, length). Bone volume deficiency can be genetically determined or change due to insufficient chewing load. Bone atrophy can also result from trauma, a past history of osteomyelitis, or endocrine and systemic diseases. Bone grafting is designed to correct this deficiency. During the surgery, the dimensions of the alveolar ridge are adjusted, and the required bone contour is created.

A sinus lift is a bone grafting procedure performed on the upper jaw.

Typically, this surgery is scheduled when planning implantation in the posterior regions of the jaw, adjacent to the maxillary sinuses. This is where the bone thickness is minimal, but the procedure allows the necessary bone volume to be built up by utilizing the internal space of the maxillary sinus. Properly performed bone grafting contributes to better osseointegration of the dental implant and ensures a strong foundation for chewing. Indications and contraindications

Indications for a sinus lift

The upper jawbone differs in structure from the lower jaw tissues. At the same time, the roots of the upper teeth are located beneath the maxillary sinuses. Securing implants there is problematic for many patients — according to statistical data, two-thirds of patients who opt for dental prosthetics require a sinus lift. Therefore, the primary indication for a sinus lift is bone atrophy. In addition, the procedure is indicated before implantation in cases such as:

anatomical structural features of the jaw;
long-term absence of teeth;

trauma with the loss of a significant portion of bone;
age-related changes.

In some individuals, the maxillary cavities are quite large. With this specific feature, an artificial tooth would simply sink into the sinus cavity during chewing. A sinus lift is effective even for this particular anatomical characteristic.

Contraindications

Like any surgical method, a sinus lift has a list of general contraindications common to all surgeries. These include:

blood clotting disorders;
heart failure;
oncology;
intolerance to anesthetics;
psychoneurological issues;
autoimmune disorders;

exacerbation of chronic diseases;
severe form of diabetes;
pregnancy and lactation;

The procedure to add bone graft material and adjust the floor of the maxillary sinus is performed using two main methods: open (lateral, major) and closed (transcrestal, minor, vertical) sinus lifts.

The balloon method is also used and is considered a variation of the closed sinus lift. This method involves the gentle detachment of the barrier mucous membrane (Schneiderian membrane) by injecting a saline solution. To achieve this, a specialized catheter is inserted into the access hole of the maxillary sinus, and the system is hermetically pressed against the sinus floor. Thanks to this technique, the detachment occurs delicately, evenly, and without the risk of damaging the sinus wall. There are also variations such as endoscopic and ultrasonic sinus lifts, but the classic methods are used most frequently.

During the operation, the surgeon creates a small hole for the implant root, shifts the base of the sinus upwards and introduces bone material into the resulting space. With the closed method of osteoplasty of the upper jaw, a bone-forming preparation is introduced through the implant bed. With closed sinus lifting, the implantation is not stretched in time, the problem of restoring the dentition is solved immediately as soon as the osseointegration of the implant root occurs.

What does preparation for surgery include?

Before osteoplasty, it is necessary to undergo an examination, which includes a laboratory blood test and an X-ray of the intervention area. This is necessary for a reliable assessment of the condition of the oral cavity and bone parameters. Air sinuses can be multi-chamber. A 3-D image taken using a computer tomograph will significantly simplify the dentist’s work, allowing you to choose the right technique and perform the operation with maximum accuracy. All this allows you to achieve the desired result. Patients who smoke are recommended to give up tobacco 10-14 days before osteoplasty and 3-4 weeks after sinus lifting.

Materials for sinus lift

Different materials are used to increase bone volume in dentistry:

Autogenous. The ideal option is the patient’s own tissue, which ensures absolute compatibility and the fastest possible regeneration rate. However, this method is more invasive and requires two surgeries — harvesting the material and the bone grafting itself.
Synthetic. Hypoallergenic bone graft substitutes that have the ability to adapt to living tissues. They create a foundation for blood vessel formation and colonization by the body’s cells, while gradually resorbing on their own.
