The Marco Rossa appliance is a fixed orthodontic device designed for maxillary expansion, particularly in children during the mixed dentition period. Its primary objective is to create sufficient space for proper eruption of permanent teeth, correct malocclusion, and prevent the need for complex orthodontic treatment in the future.
The most favorable outcomes are achieved during early childhood, when bone tissue is actively growing and the midpalatal suture is easily amenable to correction. This enables the appliance to address not only dental alignment but also overall jaw development.
What is the Marco Rossa Appliance
The Marco Rossa appliance is a custom-fabricated orthodontic device consisting of:
- an acrylic baseplate conforming to the palate;
- an orthodontic expansion screw;
- metal bands or wires for attachment to teeth;
- auxiliary retention components.
In clinical practice, you may also encounter the terms Haas appliance, Marco Rossa plate, or palatal expander — these are synonyms referring to the same device category.
Important: this is a fixed appliance. It is bonded to the teeth and functions continuously, 24 hours a day, regardless of patient compliance. The Marco Rossa appliance is not used in adults, as the design is intended for growing jaws in children.
Each appliance is custom-made based on impressions or 3D scanning. Children can choose the color of the acrylic or decorative elements, which significantly improves psychological acceptance of treatment.
The device is virtually invisible during conversation or smiling, as it is positioned inside the oral cavity.

How the Marco Rossa Appliance Works
The mechanism of action is based on controlled maxillary expansion.
Parents or the orthodontist activate the screw using a special key according to a prescribed schedule. During activation:
- the acrylic baseplate gradually separates;
- gentle, controlled pressure is applied to the dental arch and midpalatal suture;
- the maxilla expands through physiological means.
In children aged 6–9 years, the midpalatal suture has not yet fully ossified, making it responsive to orthodontic force without surgical intervention.
Maxillary expansion automatically creates proper conditions for mandibular development, bite alignment, and tongue position stabilization.
The activation frequency typically ranges from 1–2 turns per week, though the exact protocol is always determined individually by the orthodontist.
Indications for Use
The Marco Rossa appliance is indicated for the following orthodontic conditions:
- dental crowding;
- insufficient space for permanent tooth eruption;
- maxillary constriction or hypoplasia;
- posterior crossbite;
- Class II malocclusion;
- Class III malocclusion;
- impaired nasal breathing due to narrow nasal passages.
The optimal treatment age is 6–9 years, during the early mixed dentition period. At this stage, skeletal structures exhibit maximum plasticity, and correction yields stable, long-term results.
Contraindications and Limitations
In certain cases, maxillary expansion appliances are contraindicated or require preliminary preparation:
- severe systemic diseases;
- bone pathologies;
- insufficient number of teeth for reliable anchorage;
- advanced age after midpalatal suture fusion;
- acute inflammatory processes in the oral cavity;
- multiple untreated carious lesions.
Comprehensive diagnostic evaluation is mandatory before initiating treatment.
Advantages of the Marco Rossa Appliance
Key benefits of this device include:
- continuous 24/7 action independent of patient compliance;
- superior efficacy compared to removable appliances;
- facilitation of proper mandibular growth;
- normalization of tongue posture during swallowing and speech;
- creation of space for permanent dentition;
- reduction in risk of dental impaction and retention;
- improvement of nasal breathing and sinus ventilation;
- positive influence on facial symmetry and aesthetics;
- reduction in duration of future treatment with braces or aligners.
Essentially, this represents an investment in the proper development of the child’s craniofacial system.
Treatment Stages with Marco Rossa Appliance
Diagnosis and Treatment Planning
Clinical examination, CT scan or panoramic radiograph, photographic documentation, occlusal analysis, and treatment simulation are performed.
Appliance Fabrication and Cementation
The jaw expansion appliance is custom-fabricated. During cementation, the orthodontist instructs parents on activation protocols and maintenance procedures.
Active Phase and Retention
The active phase typically lasts 3–6 months. After achieving the desired expansion, the appliance remains in place for an additional period to stabilize the results (retention phase).
Patient Adaptation
The adaptation period usually takes 2–3 days.
Temporary sensations may include:
- foreign body sensation in the mouth;
- minor speech alterations;
- increased salivation;
- unusual sensations during mastication.
These symptoms resolve spontaneously and do not require special intervention.

Maintenance of the Marco Rossa Appliance
Proper oral hygiene is essential for successful treatment.
Recommendations:
- brush teeth at least twice daily;
- thoroughly clean the area around the baseplate and screw;
- rinse mouth with water after each meal;
- use an oral irrigator for deep cleaning;
- maintain regular dental visits for preventive examinations and professional prophylaxis.
Regular dental visits and professional cleanings enable timely detection of any issues, verification of proper appliance function, and maintenance of healthy teeth and gingiva throughout treatment. This level of monitoring ensures result stability and prevents the need for repeated orthodontic intervention in the future.
